SUMMARY:BRI Care Consultation Learning Collaborative – How to Leverage Partner Organizations to Implement the GUIDE Model
DESCRIPTION:REGISTER\n\n\n\n\n\n\n\nThis session will answer some of your most burning questions about how to establish an effective partnership with a Care Navigation partner – in this case between a healthcare system and a community-based organization — to deliver GUIDE Model services. \n\n\n\nWe’ll share the nuts and bolts of how Benjamin Rose\, a Partner Organization\, and Tembo Health\, a virtual provider of specialty dementia care services and Participating Site\, worked collaboratively to establish a division of responsibilities that address all GUIDE Care Delivery requirements.
